Training objective
Our training has been specifically designed to provide Link Workers and Community Connectors with the essential skills, tools, methodology, and knowledge to offer effective Social Prescribing services. We aim to develop the capacity of Social Prescribers in the UK in a bid to reduce the frequency with which patients visit primary health care services for social or economic issues.
By building a workforce of skilled Social Prescribers across the country, we can have a significant impact on the health and wellbeing of patients and take the strain on behalf of medical professionals.
Training content
As a participant in this training, you’ll learn:
- Initial patient/client interview and assessment methods.
- How to analyse the initial assessment and interview.
- The approach to planning a Social Prescription.
- Methods of implementing agreed actions.
- How to compile an extensive list of local community and public services that may be relevant to the delivery of the Social Prescribing service.
- Successful strategies for negotiating referral arrangements with relevant local service providers
- Different approaches to developing a community activity of various interests.
- How to complete follow-ups and reviews.
In addition to the training, participants will also complete two work-based assignments that will be used to assess implementation of skills and knowledge gained during the training.

By the end of the training participants should be able to demonstrate:
- The skills to effectively assess and understand the needs of referred clients and identify appropriate actions and services to address them.
- An understanding of different approaches in advocating and referring clients to other providers.
- The ability to work effectively with statutory services and voluntary organisations.
- Strategies to manage caseloads and to record actions effectively.
- An awareness of various community-based activities and resources that may be useful for the Social Prescribing service.
- The ability to develop community-based activities of interest where necessary.
